• If you are presently or were previously under any eye doctor's care, please obtain copies (or summary) of your last eye exam and operative report(s) if you have had any eye surgery. Print out as many Eyecare Records Release forms as you need, fill them out and send a copy to each of your current and/or previous eye doctors. This will authorize the release of your records directly to Sarasota Cataract & Laser Institute so that we will have them BEFORE your appointment.
